SS-31 (also known as Elamipretide, Bendavia, or MTP-131) is a synthetic, cell-permeable tetrapeptide with the unique ability to selectively accumulate in the inner mitochondrial membrane. It was developed by Prof. Hazel Szeto and Peter Schiller at Cornell Weill Medical College and is in clinical development by Stealth BioTherapeutics. Mechanism of action — cardiolipin binding
The mechanism of action of SS-31 is fundamentally different from that of most research peptides: its target is not a cell surface receptor, but cardiolipin —an anionic phospholipid unique to the inner mitochondrial membrane.
- Electrostatic guidance: the peptide’s alternative aromatic-cationic motif electrostatically attracts it to the highly negative potential of the inner mitochondrial membrane
- Binding to cardiolipin: SS-31 selectively binds to cardiolipin, thereby concentrating directly in the electron transport chain
- Stabilization of the crystal structure: prevents the peroxidation of cardiolipin → preserves the integrity of mitochondrial crystals and ETC supercomplexes
- ROS reduction at the source: Unlike antioxidants, which scavenge free radicals only after they have been generated, SS-31 prevents excessive ROS production by maintaining an efficient flow of electrons
- Increased ATP output: optimization of ETC function → higher ATP production in cells under metabolic stress
Areas of Preclinical and Clinical Research
Mitochondrial cardiomyopathy and heart failure
In a Phase 2 clinical trial for HFrEF (heart failure with reduced ejection fraction), SS-31 demonstrated a 10–15% improvement in peak VO₂ after 4 weeks of IV administration. For Barth syndrome (a genetic cardiomyopathy caused by cardiolipin defects), improvements were observed in the 6-minute walk test and cardiac stroke volume.
Renal Protection
Preclinical models of renal ischemia-reperfusion have demonstrated the preservation of mitochondrial function and a reduction in tubular apoptosis in cell cultures and animal models.
Research on Aging and Mitochondrial Dysfunction
With age, cardiolipin oxidation disrupts ETC supercomplexes—SS-31 is being investigated in animal models of aging as a tool for preserving mitochondrial biogenesis and skeletal muscle function.
Neuroprotection
In models of oxidative stress in neuronal cells, SS-31 protects against apoptosis, making it a subject of research into neurodegenerative diseases.
